Job summary

Cffde in Delaware seeks a Nurse Home Visitor to deliver comprehensive nursing services under the Nurse-Family Partnership, working with pregnant women and families up to two years old. You will conduct in-home visits, assess health needs, connect families with resources, and document progress while maintaining fidelity to the model.

Travel across Sussex County is required; the role requires flexibility, bilingual English/Spanish is desirable, RN with ADN and license, and strong communication

Qualifications

• Must be a registered nurse with an ADN and active RN license.
• Public health, community or maternal-child health experience preferred.
• Bilingual English/Spanish desirable; CPR current.
• Valid driver’s license required; home visiting experience a plus.

Responsibilities

• Provide comprehensive nursing services to women and families in the NFP program.
• Carry a caseload of 25-30 clients with in-home visits across Sussex County.
• Travel to families' homes and community locations; flexible hours including nights/weekends.
• Support parents to build strengths and connect with community resources.
• Participate in extensive model training and supervision.
