Job summary

Aurora Health Care Metro, Inc. is seeking a registered nurse to provide patient-centered care across the continuum in Colorado. You will assess needs, plan individualized interventions, and involve patients and families in the care process.

You will monitor for changes, adjust plans, collaborate with the health care team, and participate in discharge planning, quality initiatives, and ongoing professional development. Must uphold safety and ethical standards in a dynamic clinical setting.

Qualifications

• Registered Nurse license in the state of practice.
• BLS for Healthcare Providers certification from the American Heart Association.
• Completion of an accredited nursing program.
• Typically 1 year of clinical nursing experience.
• Ability to provide holistic patient care and collaborate with the care team.
• Strong communication, organization, and problem-solving skills.
• Proficiency with electronic medical records and computer systems.

Responsibilities

• Assess patient needs using the nursing process and involve family in care.
• Monitor for significant changes in patient condition and adjust the care plan.
• Implement strategies to reduce risk and increase safety, and educate patient/family.
• Coordinate discharge planning with interdisciplinary team.
• Evaluate patient and team outcomes and take action when needed.
• Communicate effectively and delegate to unlicensed staff.
• Promote safety culture and report safety events.
• Apply ethical decision making and collaborate with the care team.
• Participate in quality and process improvement initiatives.
• Pursue professional development and complete required competencies.
• Provide age-appropriate care across the lifespan.
• Maintain knowledge of clinical standards and policies.
