TEEMA is seeking a licensed Physical Therapist to provide patient-focused assessments and evidence-based interventions in a collaborative, on-site setting. You will document progress in the electronic health record, coordinate with a multidisciplinary team, and participate in quality improvement and outreach initiatives.

Responsibilities include developing care plans, forecasting outcomes, and engaging patients and families to promote adherence.

Qualifications

  • PT licensure with recent graduate eligibility and supervision plan.
  • Completion of entry-level physical therapy assessment and plan development.
  • Evidence-based PT interventions and outcome-driven treatment delivery.
  • Adherence to clinical guidelines, safety, and infection control protocols.
  • Documentation of patient progress in electronic health records.
  • Multidisciplinary collaboration and care coordination.
  • Participation in care pipelines, forecasting patient outcomes and referrals.
  • Patient and family engagement to promote adherence and education.

Responsibilities

  • Provide patient-focused PT assessments and treatment plans.
  • Deliver evidence-based PT interventions to improve functional outcomes.
  • Document patient progress and update care plans in the EHR.
  • Collaborate with multidisciplinary team to coordinate care.
  • Participate in team meetings and quality improvement initiatives.
  • Support growth targets by identifying referrals and outreach.
