Job summary

Qualifications

• New Graduate LPNs are welcome to apply.
• Level One Fingerprint Clearance Card required.
• Current LPN license (as applicable).
• AHA BLS for Healthcare Providers.
• CPI Certification required within 30 days of hire.
• Knowledge of growth and development principles, including the ability to assess, interpret data, determine patient care needs, and provide age-appropriate care.
• Understanding of legal aspects of nursing and the importance of accurate, timely documentation per standards, policies, and procedures.

Responsibilities

• Perform comprehensive nursing assessments and document findings.
• Develop, implement, and evaluate individualized treatment plans with the multidisciplinary team.
• Administer prescribed medications and monitor for adverse reactions.
• Provide crisis intervention, de‑escalation, and behavioral management per facility protocols.
• Educate patients and families on diagnosis, treatment, and continuing care.
• Monitor and document patients’ physical and emotional status; report significant changes promptly.
• Maintain a safe, therapeutic environment.
• Participate in daily shift reports and interdisciplinary team meetings.
• Follow infection control and safety guidelines at all times.
