The navigator collaborates with specialists and primary providers to optimize outcomes, address barriers to care, and improve patient satisfaction and throughput across care settings.

Qualifications

• Bachelor's degree in nursing or related field required.
• Current RN/PT/OT licensure in the state of practice.
• BLS certification required.
• Typically 5 years of clinical experience.

Responsibilities

• Provide comprehensive care coordination across inpatient and outpatient settings.
• Coordinate with physicians, nurses, and other providers to plan care.
• Educate patients and families, addressing barriers to care.
• Assist in discharge planning and transition of care.
• Support process improvement and patient satisfaction goals.
