Job summary

Beacon Health System is seeking a Neurology Nurse Navigator to support ambulatory neurology access, care coordination, and patient progression across the care continuum. The role reports to the Practice Manager/Director and serves as a key clinical-operational resource to guide patients from initial consult through diagnostics, treatment, and longitudinal management.

  • Serve as liaison among patients, families, providers, and services.

Neurology Nurse Navigator (BMG)

Description

Reports to the Practice Manager/Director. Serves as a key clinical-operational resource to supportambulatory neurology access, care coordination, and patient progression across the continuum(new consult → diagnostics → treatment → longitudinal management).

Coordinates care for patients withcomplex neurological conditions, ensuring seamless integration acrossclinic, diagnostics, infusion services, and hospital transitions. Acts asacentral pointof contact and advocatefor patients, physicians, and care teams, removing barriers to care, improving patient experience, and supportingtimelyaccess to services. Supportsclinical-operational alignmentbyoptimizingscheduling, reducing delays in diagnostics/treatment, and promoting adherence to evidence-based care pathways.

Responsibilities
Care Navigation & Patient Access
  • Guide patients through the ambulatory neurology care pathway frominitialreferral through ongoing disease management
  • Conduct initial patient outreach to:
  • Assess urgency, symptom burden, and barriers to care
  • Ensureappropriate leveland timing of visit
  • Coordinatetimelyscheduling of consults, procedures, and diagnostics
  • Reduce leakage and delays by actively managinginternal referrals and external transitions
  • Clinical Coordination & Patient Flow
  • Collaborate with neurologists, APCs, MAs, and referral teams to ensureefficient patient progression
  • Serve as liaison between:
  • Patients/families
  • Providers
  • Ancillary services (therapy, infusion, imaging)
  • Supporthospital-to-clinic transitions
  • Monitor and address gaps in care that couldimpactclinical outcomes or access timelines
Chronic Disease & High-Risk Patient Management
  • Proactively manage patients withhigh-risk or complex neurologic conditions, including:
  • Frequent EDutilization
  • Missed appointments or care gaps
  • Disease instability
  • Reinforce care plans, medication adherence, and follow-up compliance
  • Coordinate multidisciplinary support (PT/OT/ST, behavioral health, social services)
Patient & Family Education
  • Provide education on:
  • Diagnosis and disease progression
  • Treatment plans (including specialty medications, infusions)
  • Safety considerations (e.g., seizure precautions, fall risk, stroke warning signs)
  • Ensure understanding ofwhen and how to access care appropriately(clinic vs. ED)
Barrier Resolution & Resource Coordination
  • Identifyand mitigate barriers such as:
  • Prior authorizations for imaging/medications
  • Transportation and access limitations
  • Financial constraints
  • Coordinate with internal teams (financial counselors, prior auth, scheduling) toexpeditecare
Quality, Outcomes & Performance Improvement
  • Track and report on:
  • Access metrics (time to new patient visit, time to diagnostics)
  • Care gaps and no-show trends
  • Patient outcomes for targeted populations
  • Participate inpractice-based quality improvement initiatives,including:
  • Access optimization
  • Care pathway standardization
  • Reduction in ED utilization/readmissions for neurology patients
Multidisciplinary Clinic & Program Support
  • Support coordination of subspecialty clinics (e.g., stroke follow-up, MS clinic, movement disorders program)
  • Ensure readiness for clinic sessions (records, diagnostics completed, care plans aligned)
  • Assistwith development of structured care pathways and protocols

Performs other functions to maintain personal competence and contribute to the overall effectiveness of the department by:

  • Completing other job-related assignments and special projects as directed.
Organizational Responsibilities
  • Attends and participates in department meetings and is accountable for all information shared.
  • Completes mandatory education, annual competencies and department specific education within established timeframes.
  • Completes annual employee health requirements within established timeframes.
  • Maintains license/certification, registration in good standing throughout fiscal year.
  • Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
  • Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
  • Adheres to regulatory agency requirements, survey process and compliance.
  • Complies with established organization and department policies.
  • Available to work overtime in addition to working additional or other shifts and schedules when required.

Education & Experience
  • RN license in Indianarequired
  • Neurology experience preferred
  • Minimum3–5 years clinical experience, ambulatory or care coordination experience strongly preferred
  • Experience withcare navigation, case management, or population health preferred
