Job summary

Beacon Health System is seeking a Neurology Nurse Navigator to support ambulatory neurology access, care coordination, and patient progression across the care continuum. The role reports to the Practice Manager/Director and serves as a key clinical-operational resource to guide patients from initial consult through diagnostics, treatment, and longitudinal management.

Qualifications

• RN licensed in Indiana with CPR/BCLS certifications.
• 3–5 years of clinical experience, ambulatory care or care coordination preferred.
• Experience coordinating neurology care pathways and patient access.

Responsibilities

• Coordinate neurology care from referral to ongoing management.
• Serve as liaison among patients, families, providers, and services.
• Optimize scheduling and transitions across clinic and hospital services.
• Educate patients and families on disease progression and treatment plans.
