Job summary

Beacon Health System in Indiana is seeking a Neurology Care Navigator to support ambulatory neurology access and care coordination. You will act as a central contact for patients, families, and care teams, ensuring timely visits, tests, and transitions across clinic, diagnostics, infusion services, and hospital settings.

You will collaborate with neurologists, MAs, and referral teams to optimize scheduling, address barriers to care, and reinforce evidence-based care pathways, including education

Qualifications

• RN license in Indiana is required.
• Neurology experience preferred.
• Minimum 3-5 years clinical experience, ambulatory or care coordination strongly preferred.
• Experience with care navigation, case management, or population health preferred.

Responsibilities

• Guide patients through the ambulatory neurology care pathway from referral through ongoing disease management.
• Assess urgency, barriers to care, and ensure appropriate visit timing.
• Coordinate scheduling of consults, procedures, and diagnostics.
• Reduce delays by managing internal referrals and external transitions.
• Collaborate with providers and services to ensure efficient progression.
• Support care pathways and protocol development; participate in quality improvemen t.
