Job summary

Collage Home and Community Rehabilitation Partners is seeking experienced Occupational Therapists for PRN opportunities in San Jacinto, CA and surrounding communities. Therapists will provide patient-centered care in patients' homes and communities, developing integrated treatment plans through interdisciplinary collaboration.

The role emphasizes hands-on evaluation and treatment of upper extremity function, ADLs/IADLs, cognitive needs and adaptive equipment.

Qualifications

• Completion of Bachelor's degree in Occupational Therapy and state licensure is required.
• Professional certification or Master's degree in Occupational Therapy is preferred.
• Experience in treatment of neurologically related disorders is strongly preferred.
• Excellent planning, organization, writing and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

Responsibilities

• Evaluation and treatment of impairments including upper extremity function, ADLs and IADLs.
• Address visual and perceptual skills, cognitive needs, adaptive equipment and assistive technology needs.
• Collaborate with interdisciplinary team to create patient-centered home-based treatment plans.
