Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a licensed Physical Therapist to deliver rehabilitation in home and community settings across Pasadena, CA. You will conduct evaluations, design treatment plans, and work autonomously within a collaborative therapy team.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current BLS/CPR certification as required by state regulations.
• Strong communication and professional interaction skills.
• Ability to lift up to 100 pounds with safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Reassess treatment effectiveness and attend staffing/family conferences.
• Perform discharge assessments and provide home/community safety guidance.
• Communicate patient needs and progress to the treatment team and family.
• Coordinate with other disciplines to improve patient outcomes.
