Job summary

Collage Rehabilitation Partners seeks a compassionate Direct Support Professional to support adults with brain injuries in a home-like setting in Riverton, Utah. You will assist with daily living activities, monitor wellness, and accompany clients to appointments, partnering with a dedicated care team to promote safety and independence.

Part-time schedule with day, evening, or overnight shifts and a requirement to work a consistent weekend day.

Qualifications

• Education: High school diploma or equivalent required.
• Physical Demands: Ability to lift up to 50lbs.
• Driving Requirements: Valid driver's license with ability to travel up to 20% of time.

Responsibilities

• Promote Independence: Assist clients with ADLs and rehabilitation goals.
• Health & Wellness: Support medications, nutrition, and meal prep.
• Proactive Behavioral Support: Implement behavior plans while maintaining boundaries.
• Observation & Advocacy: Monitor changes and alert the team for timely care.
• Transportation & Outings: Safely transport clients to appointments and activities.
