Qualifications

• Ability to lift up to 50 lbs.
• Valid driver’s license and ability to travel up to 20%.

Responsibilities

• Assist with daily living activities and independence goals.
• Support rehabilitation activities and motivate engagement.
• Complete shift documentation and data sheets per standards.
• Transport clients to appointments and group activities as needed.
