Job summary

Learning Services, A Collage Rehabilitation Partner in Riverton, UT, is seeking a compassionate Direct Support Professional (DSP) to help adults progress toward independence in rehabilitation settings.

In this part-time role, you will assist with daily living activities, support health and wellness, and collaborate with care teams to ensure safe, respectful care. You will also document care, accompany clients to appointments, and promote a safe home-like environment.

Qualifications

• High school diploma or equivalent required.
• Ability to lift up to 50lbs.
• Valid driver’s license with a record of responsible driving behavior. Must have the ability to travel and/or operate a vehicle for up to 20% of the time spent working in this position.

Responsibilities

• Promote Independence: Assist, guide, and encourage clients with Activities of Daily Living (ADLs), community living skills, and personalized rehabilitation goals.
• Health & Wellness: Assist with medication administration as assigned, monitor nutrition plans, and prepare well-balanced meals daily.
• Proactive Behavioral Support: Support clients through challenging situations and implement behavioral management plans while maintaining professional boundaries and mutual trust.
• Observation & Advocacy: Identify changes in clients' behavioral, emotional, cognitive, or physical conditions and alert the treatment team to ensure timely care.
• Transportation & Community Outings: Safely transport and accompany clients to medical appointments, group outings, errands, and recreational activities.
• Compliance & Record Keeping: Accurately complete required shift documentation—including daily data sheets, goal sheets, vital/seizure flow sheets, supervision checklists, and incident reports in compliance with CARF, state, and federal standards.
• Environment & Safety: Maintain a clean, organized, safe, and welcoming living environment for residents in alignment with licensing requirements.
• Team Communication: Participate in shift hand-offs, review operational communications, and collaborate effectively with family members, care teams, and leadership.]
