Qualifications

• Graduate from accredited nursing program (ADN, ASN, BSN)
• Active RN license in NC in good standing
• Current CPR and First Aid Certification
• Valid driver’s license with reliable transportation

Responsibilities

• Develop and implement nursing care plans for adult and geriatric patients.
• Perform Chronic Care Management activities and assist with patient intake.
• Obtain and record patient vitals and document allergies/medications in the EMR.
• Document patient interactions and care changes in the EMR with accuracy.
• Support patient inquiries, hospital calls, lab results, and prescription refills as needed.
