Job summary

Live Well Homecare is seeking an Outpatient Registered Nurse (RN CAP 2) to provide comprehensive, clinically competent care to patients receiving hemodialysis within the ESKD team. This role includes assessment, planning, intervention, and evaluation of patient care in a float capacity.

The position involves delegating tasks to care staff, teaching, leading the team, and participating in quality improvement efforts. Travel to training sites or other facilities may be required.

Qualifications

• Requires graduation from an accredited nursing program.
• Current state RN licensure.
• CPR/BLS certification.
• Minimum 2 years of nephrology nursing experience.
• Ability to work in a health care environment with exposure to infectious materials.
• Demonstrated leadership capabilities and adherence to policies and regulations.

Responsibilities

• Collects patient data before, during, and after hemodialysis and adjusts treatment plans accordingly.
• Documents in medical records and educates patients and families.
• Administers medications and responds to emergencies.
• Ensures safe delivery of hemodialysis therapy and maintains lab procedures.
• Leads, mentors, and role models staff; participates in quality improvement activities.
• May serve as a preceptor and oversee patient care staff.
