Job summary

Mercy Hospital in Durango, CO seeks an Advanced Practice Provider in Nephrology to join a collaborative team delivering high-value kidney care across the Four Corners region. You will evaluate, diagnose, and manage CKD, AKI, hypertension, electrolytes, and ESRD with nephrologists, PCPs, hospitalists, and dialysis teams.

The role requires board-certified NP or PA, compassionate care, and a commitment to patient-centered outcomes within our mission-driven, community-focused setting.

Qualifications

• Master's degree in Nursing as NP from an accredited program.
• RN license in participating state; CO PA license or eligibility.
• Board certification as an Advanced Practice Nurse (APN).
• Prescriptive authority and BLS/ACLS where applicable.

Responsibilities

• Evaluate, diagnose, and manage CKD, AKI, hypertension, electrolytes, and ESRD.
• Collaborate with nephrologists, PCPs, hospitalists, dialysis teams, and nurses.
• Educate patients on kidney disease management and plan individualized care.
• Contribute to continuity of care and program expansion across the region.
