Job summary

Devoted Health seeks a remote Advanced Practice Provider (NP/PA) specializing in Chronic Kidney Disease to deliver virtual nephrology care for CKD patients. You will optimize medications, manage electrolytes, and coordinate care plans with a multidisciplinary team across telehealth visits.

The role requires NP/PA licensure, BLS, and at least 5 years in outpatient practice with 2+ years in nephrology, plus comfort with telehealth and EHR systems. Fluency in Spanish is a plus.

Qualifications

Must possess a current, active NP or PA license in at least one U.S. state.
Active BLS at time of hire and maintained during employment.
5+ years in outpatient clinical practice.
2+ years in nephrology or dialysis.
Proficiency with telehealth and electronic health records (EHR).

Responsibilities

Deliver direct nephrology care to CKD patients and optimize medications.
Manage electrolytes and volume status; support goals of care and dialysis planning.
Collaborate with physicians, social workers, pharmacists, and care team.
Interface with families and caregivers to coordinate holistic care.
Participate in panel reviews and quality improvement activities.
