Qualifications

• Master’s or Doctorate in Nursing (MSN or DNP) required.
• Current licensure as a Nurse Practitioner in New Mexico (or eligibility).
• National board certification (AANP, ANCC, or equivalent).
• DEA license and prescriptive authority.
• Experience in outpatient nephrology clinics and dialysis settings.
• Hospital rounding experience as applicable.
• CME allowance & professional development support.
• Malpractice coverage.

Responsibilities

• Deliver high-quality clinical care across outpatient clinics, dialysis units, and hospital settings.
• Develop proactive care plans to slow disease progression and optimize medications.
• Provide timely follow-up and continuity of care.
• Support transitions between clinic, hospital, and dialysis settings.
• Educate patients and families on kidney disease, treatment options, and lifestyle changes.
• Counsel on dialysis modalities and transplant readiness.
• Support patients’ emotional and psychosocial needs.
• Conduct dialysis rounds (in-center and home modalities).
• Collaborate with nephrologists, nurses, dietitians, and social workers.
• Coordinate care for patients with diabetes, hypertension, and other comorbidities.
