Job summary

U.S. Renal Care in Laredo, TX seeks a Family/Adult Nurse Practitioner to manage kidney disease care in collaboration with nephrology and dialysis teams. You will conduct annual and on-going care, coordinate hospital transitions, and promote chronic disease management for improved outcomes.

Responsibilities include exams, ordering tests, prescribing meds, EMR documentation, and patient education. Bilingual in English/Spanish a plus; on-site residency in Laredo required.

Qualifications

• Licensed Advanced Practitioner in Texas required.
• 3+ years of direct patient care experience preferred.
• Spanish bilingual a plus, strong English communication.

Responsibilities

• Provide holistic patient care management for kidney disease patients.
• Collaborate with nephrologists, dialysis teams, and other providers.
• Conduct disease management visits and transitional care.
• Perform examinations, order tests, and prescribe medications.
• Document in EMR and educate patients and families.
