Job summary

DaVita IKC seeks a passionate Nurse Practitioner to join our Port Charlotte/Sarasota, FL team. The NP will perform Comprehensive Health Evaluations, manage CKD/ESKD patients, and collaborate with nephrologists to optimize outcomes.

The role involves occasional telehealth and travel across the assigned geographic area, with a Monday–Friday schedule and no nights, weekends, or on-call. Competitive pay, excellent benefits, CME reimbursement, license renewals, and further professional growth

Qualifications

• Current, unrestricted NP license in state of practice; DEA license or eligibility within 90 days.
• Current CPR/BLS certification.
• Valid driver's license and insurance in the state.
• Intermediate computer skills (MS Word, Excel, Outlook).

Responsibilities

• Complete Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions.
• Manage CKD and ESKD patients, focusing on slowing disease progression and preventing readmissions.
• Utilize prescriptive authority and medication management in collaboration with nephrologists.
• Lead the clinical team to ensure care coordination aligns with medical treatment plans.
