Qualifications

• NP or PA with 5+ years in outpatient nephrology
• Minimum 2 years in nephrology or dialysis
• Proficiency with telehealth and EHRs
• Spanish language skills desired
• Current, active NP/PA license in at least one US state

Responsibilities

• Deliver direct nephrology care to CKD patients and optimize meds
• Collaborate with nephrology team including physicians, social workers, pharmacists
• Coordinate holistic care with families and caregivers
• Participate in panel reviews and medical management guidance
• Perform comprehensive assessments and quality-of-care reviews
• Use home grown EHR system and provide interface feedback
• Maintain accurate patient records and ensure regulatory compliance
• Engage in quality improvement and professional development
• Adhere to telehealth regulations and privacy laws
