Job summary

Devoted Medical Group, PC is seeking a nurse practitioner or physician assistant with nephrology experience to join a virtual-first CKD clinic serving seniors. You will optimize medications, manage electrolytes, and coordinate care with the nephrology team, social workers, and caregivers to deliver high-quality, patient-centered care.

You will work with telehealth platforms and EHR systems, ensuring compliant documentation and proactive care planning.

Qualifications

• 5+ years in outpatient clinical practice (nephrology preferred).
• Experience in nephrology management and dialysis planning.
• Telehealth experience and virtual care familiarity.
• Spanish language ability is a plus.

Responsibilities

• Deliver direct nephrology care to CKD patients, optimize medications, manage electrolytes and volumes.
• Collaborate with nephrology team, social workers, pharmacists, and clinical staff.
• Coordinate with families to ensure continuity of care and holistic treatment plans.
• Participate in panel reviews and quality improvement initiatives.
