Responsibilities

• Administer, record, and track PAM tool to patients or caregiver (25%).
• Collect, track, and record patient information related to dialysis starts (20%).
• Track, collect, and communicate data for patients at risk for end stage renal disease (25%).
• Collect, track, and communicate information related to kidney disease education (20%).
• Other duties as assigned (10%).
