Eastern Nephrology Associates (ENA) seeks a Greenville East, NC-based care coordinator to support patient care by collecting and tracking information, PAMs, dialysis starts, and education data. This full-time clerical role emphasizes accuracy, confidentiality, and teamwork.

The position is on-site at ENA, with a Monday–Friday schedule, occasional after-hours and travel to other ENA sites as needed. You will use MS Office, SharePoint, and data systems to document and report findings to the care

Qualifikationen

• ADN or related clinical field with 2 years' experience.
• HIPAA confidentiality requirements.
• Valid driver's license and reliable transportation.

Aufgaben

• Administer, record, and track PAM tool to patients or caregivers 25%.
• Collect, track, and record patient information relating to dialysis starts 20%.
• Track, collect and communicate data for patients at risk for ESRD 25%.
• Collect, track, and communicate information relating to kidney disease education 20%.
• Other duties as assigned 10%.
