UNC Health Care is looking for a Nurse Coordinator to join the Eastowne Nephrology Clinic in Chapel Hill, NC. The role is 100% onsite, 40 hours/week, with a daytime schedule and no weekends or holidays.

The position supports clinical nursing care coordination, discharge planning, and patient education in a dynamic outpatient nephrology setting. Responsibilities include rounds with the care team, data collection for performance metrics, and coordinating post-hospitalization care and referrals.

Formación

• RN license in NC required.
• Graduation from a state-accredited nursing program.
• Minimum 5 years of professional nursing experience.

Responsabilidades

• Assess patient status with an interdisciplinary team to determine care needs.
• Coordinate patient care and discharge planning across care settings.
• Collect and analyze data for performance improvement and outcomes.
• Promote patient education; document care plans and teaching in medical records.
• Attend care conferences and coordinate post-hospitalization care and referrals.
