Vigilant Healthcare Solutions LLC is seeking a Board-Certified Nephrology Physician to join a collaborative medical team in a long-term onsite opportunity. You will provide comprehensive nephrology care in both inpatient and outpatient settings within a modern clinical environment focused on safety and high-quality outcomes.

The role includes managing AKI, dialysis oversight, and coordinating with multidisciplinary teams.

Qualifications

• Board Certified or BE in Nephrology per ABIM or AOBIM.
• MD or DO with an ABIM/AOA nephrology fellowship after internal medicine residency.
• Current, full, unrestricted medical license in the U.S. and active DEA registration.
• Active ACLS and BLS certifications.
• U.S. citizenship and credentialing/privileging clearance.

Responsibilities

• Manage inpatient nephrology consultations and AKI evaluation with dialysis oversight.
• Conduct outpatient clinic evaluations and longitudinal disease management for nephrology patients.
• Provide supervision and prescription management for hemodialysis services and assess dialysis access.
• Participate in call rotations evenings/weekends/holidays to support renal emergencies.
• Coordinate care with primary teams and specialists; document in the EHR.

Job description

Vigilant Healthcare Solutions is seeking a Board-Certified Nephrology Physician to join a collaborative and highly respected medical team. This is a long-term onsite opportunity in a modern, well-equipped clinical environment where patient safety and quality of care are top priorities.

The physician will provide comprehensive nephrology care in both outpatient and inpatient settings, working alongside an experienced multidisciplinary care team to deliver high-quality patient outcomes.

Responsibilities

• Manage inpatient nephrology consultations, evaluate acute kidney injury (AKI), fluid/electrolyte disorders, and acid-base disturbances; direct acute hemodialysis or continuous renal replacement therapy (CRRT) in critical care units
• Conduct outpatient clinic evaluations and longitudinal disease management for patients with hypertension, glomerular diseases, diabetic nephropathy, and pre-dialysis chronic kidney disease
• Provide medical supervision and prescription management for outpatient and inpatient hemodialysis services, including evaluation of dialysis access (fistulas, grafts, catheters) and dialysis adequacy
• Participate in scheduled weeknight, weekend, and holiday call rotations to support emergency renal consultations and urgent dialysis procedures
• Coordinate care huddles with primary care teams, vascular surgeons, intensivists, dietitians, and renal social workers to optimize patient outcomes
• Complete timely, detailed progress notes, consultation reports, procedure notes, and dialysis orders within the facility's electronic health record system

Qualifications

• Board Certified (BC) or actively Board Eligible (BE) in Nephrology through the American Board of Internal Medicine (ABIM) or American Osteopathic Board of Internal Medicine (AOBIM)
• MD or DO from an accredited program, with completion of an ACGME- or AOA-accredited Internal Medicine Residency followed by a Nephrology Fellowship
• Current, full, active, unrestricted medical license in any U.S. State, Territory, or Commonwealth, plus active, unrestricted DEA registration
• Active ACLS and BLS certification through the American Heart Association
• Demonstrated clinical competency in inpatient/outpatient nephrology and hemodialysis oversight
• Must be a U.S. Citizen and able to pass a standard background investigation; must complete the facility's credentialing and privileging process prior to providing patient care

Schedule

• Clinic Hours: Mon day - Friday, standard daytime clinic hours (100% onsite)
• Coverage provided by a rotating pool of physicians to maintain daily onsite presence
• Rotating call coverage for evenings, weekends, and holidays
