Qualifications

Responsibilities

• Obtain medical history and perform physical exams; develop differential diagnoses and treatment plans.
• Prescribe or order evidence-based pharmacologic therapies and follow-up.
• Provide education to patients and families; promote continuity of care and care coordination.
