Community Bridges, Inc. (CBI) is seeking a Jail Coordinator for Navajo County to deliver treatment and discharge planning to jail patients, coordinating into behavioral health services after release.

The role involves collaborating with probation and jail staff, participating in weekly staffing, and ensuring adherence to DHS, RBHA, ACC OBHL, and CFR guidelines while supporting patients’ recovery and continuity of care.

Qualifications

• 2 years related experience in behavioral healthcare, physical health or substance abuse.

Responsibilities

• Deliver treatment and discharge planning to identified Jail Program Patients.
• Assist inmates with jail release planning and coordinates into behavioral health services.
• Participate in weekly staffing with probation and Jail staff to provide updates on each patient and coordinate care.
