Job summary

Vita of Greenfield is seeking a compassionate Social Services Coordinator to support residents and families across three communities. The role focuses on Medicaid eligibility, applications, redeterminations, spend-downs, and Waiver programs, while also assisting with social services needs.

Responsibilities include completing Medicaid paperwork, coordinating with DFR/Maximus, educating residents, and maintaining HIPAA-compliant records.

Qualifications

Experience handling Medicaid eligibility and spend-down processes.
Ability to manage multiple locations with discretion and organization.
Strong communication and customer service skills.

Responsibilities

Complete, submit, and track Medicaid applications, redeterminations, and spend-downs.
Coordinate with residents, families, DFR, Maximus, and aging agencies.
Monitor pending cases and follow up on documentation to prevent lapses.
Report income/resources and patient liability in coordination with the Business Office.
Educate residents and families about Medicaid eligibility, waivers, apps, and appeals.
Assist Admissions with Medicaid documentation for new residents.
Address grievances and ensure proper follow-up.
Maintain confidential resident records per HIPAA and regulations.
Travel regularly between Greenfield, Union City, and Yorktown.
Participate in meetings, trainings, and required in-services.
Perform other duties as assigned.
