Job summary

Advocate Health seeks a Care Management Leader to oversee daily operations for care management and social work at multiple sites. You will supervise staff, align performance with regulatory requirements, and collaborate with physician leadership to improve outcomes.

The role requires RN or Social Worker licensure with relevant certifications within 1 year, and strong leadership in care coordination and budget management. Travel across IL/WI sites may be required.

Qualifications

• Requires RN or Social Worker licensure in the practicing state.
• ACM/ACM certification within 1 year or CCM/CMSCC certification within 1 year.
• RN-BC Nursing Case Management certification within 1 year; C-SWHC within 1 year.

Responsibilities

• Oversee daily operations of care management and social work at assigned site(s).
• Approve staff schedules, PTO, and time cards to ensure continuity of services.
• Develop performance measures, targets, and benchmarks for care processes.
• Collaborate with medical leadership to review physician trends and improvement plans.
• Act as internal consultant on care management opportunities and educate teams.
• Manage budgets and control expenditures within approved objectives.
• Ensure CMS/OSHA/Accrediting standards compliance and ethics.
