Job summary

Advocate Health in Wisconsin/Illinois seeks a care management leader to oversee daily operations of care management and social work across assigned sites.

You will monitor staff schedules, drive performance improvements, and ensure regulatory compliance with CMS/OSHA standards. Requires RN or SW licensure and CCM/ACM or CCMC certification within 1 year, with travel across sites as needed.

Qualifications

• RN or SW license as required.
• ACM/CCM or CCM certification within 1 year (ACM/CCM) or CCMC within 1 year.
• Knowledge of CMS/OSHA/regulatory requirements.

Responsibilities

• Oversees daily operations of care management and social work at assigned sites.
• Monitors staff schedules, PTO, and time cards to ensure continuity of services.
