Job summary

Humana is seeking a Manager, Care Management to lead a multidisciplinary team serving Medicare/Medicaid members in Illinois. You will guide care planning, leverage extenders like CHWs, and collaborate with community partners to improve outcomes and reduce disparities.

The role requires clinical or social work licensure, a track record in care management leadership, and strong data-driven decision making. Travel will be frequent, with 40-hour weeks and remote work options available.

Qualifications

• Active RN or Social Work license required.
• Associate's Degree required; Bachelor's preferred.
• 5+ years of professional experience.
• 2+ years of management or supervisory experience.
• Strong data analysis, communication and problem-solving skills.
• Knowledge of Medicaid/Medicare and long-term care guidelines.
• Proficiency with Windows, Outlook, Excel, Word and MIS software.

Responsibilities

• Oversee assessment and evaluation of members' needs to support wellness.
• Lead cross-functional care management teams and external partners.
• Ensure compliance with federal, state, and contract requirements.
• Coordinate care planning and service delivery across departments.
• Monitor program performance and drive continuous improvement.
• Develop and maintain policies to support high-quality service.
