Job summary

Outcomes Operating Inc. seeks a Clinical MTM Pharmacist to provide patient-centered MTM services aimed at optimizing medication therapy and improving outcomes.

The role includes Comprehensive Medication Reviews, targeted interventions, adherence monitoring, and transitions in care, delivered via telehealth as needed. Candidates should hold a PharmD or Bachelor of Pharmacy and a current pharmacist license with ability to obtain licensure in multiple states.

Qualifications

  • Professional Degree in Pharmacy (PharmD or B.Pharm) and active pharmacist license.
  • Ability to obtain licensure in multiple states as needed.

Responsibilities

  • Deliver MTM services including Comprehensive Medication Reviews and Medication Therapy Management interventions.
  • Communicate clinical recommendations to prescribers and care teams in a clear, timely manner.
  • Document patient interactions and therapeutic recommendations in the medical record with HIPAA compliance.
  • Provide patient-centered education and support for chronic disease management.

JOB SUMMARY (added Nov 2023)The Clinical Medication Therapy Management (MTM) Pharmacist will provide patient-centered MTM services for the purpose of optimizing medication therapy to improve clinical, economic and humanistic outcomes. The Clinical MTM Pharmacist will participate in a broad spectrum of MTM services including, but not limited to, Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and Transitions in Care to help achieve these outcomes. MTM services may be provided to patients telephonically or by remote video link (telehealth)ESSENTIAL DUTIES & RESPONSIBILITIESDeliver MTM services, including but not limited to, Comprehensive Medication Reviews, Disease Specific Medication Reviews, and Targeted Interventions to assess appropriateness of all medications, to optimize medication therapy and to improve clinical outcomesCommunicate and document clinical recommendations to prescribers and interdisciplinary care teamsKNOWLEDGE & REQUIREMENTSKnowledge and understanding of the 5 Core Elements of MTM services – 1) Medication Therapy Review – Comprehensive and Targeted, 2) Personal Medication List (PML), 3) Medication-Related Action Plan (MAP), 4) Interventions and Referrals, and 5) Documentation of Services and Follow-upAbility to efficiently assess prescribed medication therapy for unnecessary medications, need for additional medications, ineffective medications, inappropriate doses, adverse drug reactions, and adherence concernsAbility to efficiently propose appropriate interventions to resolve medication therapy problems, achieve goals of therapy and prevent new medication therapy problemsAbility to assist with managing medication costs through cost-effective interventionsAbility to help patients identify medication adherence barriers and develop solutions to overcome those barriersAbility to efficiently evaluate and accurately document patient care notes, prior adverse reactions, patient recommendations, and prescriber recommendations in a clear, concise, professional and timely mannerAbility to collaborate with prescribers, interdisciplinary care teams, patients and/or caregivers to identify and develop patient-specific disease state therapy goalsAbility to provide patient-centered education on chronic disease states, including medications used for prevention and treatmentStrong Telephonic Skills – Ability to speak clearly and communicate fluidly with senior REQUIREMENTSMinimum years of work experience: 1 yearMinimum level of education or education/experience: Bachelors Degree in Pharmacy or PharmD from a college accredited by ACPE or graduate of a foreign college of pharmacyCurrent license to practice Pharmacy in the state(s) you are practicing inAbility to acquire pharmacist licensure in multiple states, if requestedCompletion of a MTM certificate coursePreferred:Board Certification in Ambulatory Care, Geriatrics, Pharmacotherapy, Medication Therapy Management, or other Board Certification provided through American College of Clinical PharmacyCompletion of an ASHP-accredited pharmacy residency and/or fellowshipPharmacist licensure in multiple statesMotivational Interviewing SkillsPHYSICAL REQUIREMENTSThe physical demands
