Formación

• Must have a master's degree from a CSWE-accredited MSW program.
• Must be certified or licensed in the state where they practice.
• At least one year of social work experience in a health care setting.
• Basic technology skills, including operation of a mobile device.
• Florida-specific: No state licensure required, certification only; must provide masters degree transcript.
• Medicare home health or hospice experience is preferred.

Responsabilidades

• Administer social services to patients requiring intermittent professional services and educate the patient, family, and other members of the health care team.
• Perform services in accordance with the physician's orders and the established plan of care (POC).
