Job summary

Enhabit Home Health & Hospice is seeking a qualified social worker to provide intermittent professional services to patients in the home, working with families and a care team to implement plans of care.

The role requires an MSW from a CSWE-accredited school, active LMSW licensure, and at least one year in a health care setting. Candidates must be licensed in Georgia and possess solid technology skills for mobile devices and communications.

Qualifications

• Master of Social Work from a CSWE-accredited program.
• Must be certified or licensed in the state where they practice.
• Minimum one year of social work experience in a health care setting.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
