Job summary

Enhabit Home Health & Hospice is seeking a licensed MSW to provide intermittent social work services to patients and families in a home health setting. You will collaborate with physicians, nurses, and caregivers to implement the plan of care and educate families on care options and resources.

The role requires an MSW with state licensure and at least one year of health care experience, plus basic tech skills to document care and communicate with the care team.

Qualifications

• Must possess a Master of Social Work degree.
• Must be certified or licensed in the state where they practice.
• Must have at least one year of social work experience in a health care setting.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
