Job summary

Community Health Systems, Inc. is seeking a Social Worker with an MSW to provide clinical social work services at Oro Valley Medical Center.

You will assess psychosocial needs, offer counseling, assist with care coordination, and connect patients and families to community resources throughout the healthcare continuum. Qualifications

• MSW from CSWE-accredited program.
• 2–4 years of clinical social work experience in healthcare setting preferred.

Responsibilities

• Conducts age-appropriate psychosocial assessments across all ages to evaluate needs.
• Provides counseling and resource education to help patients and families cope with illness and transitions.
• Participates in discharge planning by identifying barriers and coordinating post-acute care.
• Facilitates referrals to internal and external services including mental health and social support programs.
• Manages high-risk psychosocial cases and crisis situations.
• Maintains timely documentation in the medical record per standards.
• Supports peers and mentors other social workers.
