Job summary

You will collaborate with the care team to promote safe transitions and improve patient outcomes across the care continuum. The role requires a BSW and strong communication skills, with healthcare experience preferred.

Qualifications

• Knowledge of social work principles, basic case coordination, and psychosocial support practices.
• Familiarity with healthcare systems and community-based resources.
• Strong verbal and written communication skills.

Responsibilities

• Conduct psychosocial screenings for patients across the age spectrum, including neonatal, pediatric, adolescent, adult, and geriatric populations.
• Identify emotional, social, and financial needs of patients and families and assists in providing appropriate support or community referrals.
• Supports the development and implementation of discharge plans in collaboration with licensed social workers and the case management team.
• Coordinates referrals for services such as home health, long-term care, rehabilitation, or behavioral health resources as directed.
• Assists in follow-up planning and facilitates communication with external providers, support agencies, and outpatient services.
• Provides emotional support to patients and families facing illness, hospitalization, or complex discharge needs.
• Participates in daily interdisciplinary care team discussions and supports case review and transition planning.
• Maintains accurate, timely documentation in the medical record in accordance with organizational policies and regulatory requirements.
• Maintains knowledge of community resources and helps connect patients to financial assistance, transportation, counseling, and support services.
• Performs other duties as assigned.
• Maintains regular and reliable attendance.
• Complies with all policies and standards.
