Responsibilities

• Participates in interdisciplinary team’s initial assessments and care planning.
• Maintains current written case management records and periodic reassessments.
• Refers participants and families to appropriate community agencies or facilities and acts as liaison.
• Encourages effective relationships among staff for cohesive teamwork.
• Consults with staff about social, emotional, and cultural factors affecting health and care.
• Participates in program and policy development of the Social Work component of Florida PACE Centers.
• Attends meetings as needed.
