Job summary

Miami Jewish Health in Miami, FL, is seeking a Social Worker (MSW) to advocate for residents and families in our long‑term care and rehabilitation setting, delivering empathetic, high‑quality social work services.

Responsibilities include psychosocial assessments, discharge planning, care coordination with interdisciplinary teams, and supporting residents’ rights and well‑being. Requires a Master’s in clinical social work and at least three years of related experience.

• Three years of related experience.
• Strong assessment and counseling skills.

Responsibilities

• Supports, counsels, and acts as an advocate for residents and family.
• Completes psychosocial assessments, treatment plans, quarterly evaluations, MDS, discharge plans, etc.
• Coordinate and facilitate patient discharges and post-discharge services.
• Collaborates with interdisciplinary staff to share resident information.
• Assists with discharge planning in a rehab setting.
• Facilitates support groups and attends meetings as needed.
