Qualifications

• 1+ years of experience or MSW internship required.
• Strong analytical thinking and ability to break down data into component parts.

Responsibilities

• Address barriers to discharge and recovery, including financial and emotional issues.
• Perform thorough assessments and develop discharge plans for high-risk patients.
• Provide emotional support during complex issues like end-of-life discussions.
