Job summary

Jupiter Medical Center is seeking an MSW Social Worker to coordinate discharge planning and provide psychosocial support across transitions of care. You will work with DME, SNF, LTAC and home health agencies to craft detailed discharge plans and ensure regulatory compliance while maintaining patient-centered communication.

The role emphasizes collaboration with physicians, nursing and case management, along with strong organizational and documentation skills to support safe, timely transitions

Qualifications

• Master’s degree in social work required.
• ACM Social Worker certification preferred.
• BLS certification for Healthcare Providers required/preferred.
• Highly organized, empathetic and capable of multitasking in a high-stress environment.
• Knowledge of community resources and the healthcare system.
• Ability to assess psychosocial needs and barriers to transition planning.

Responsibilities

• Provide psychosocial assessments and interventions for acute care patients and families.
• Coordinate discharge planning with DME, SNF, LTAC and home health providers.
• Craft comprehensive discharge plans including destination, transportation and follow-up care.
• Discuss plans with patient, family and care team; document in medical records.
• Maintain knowledge of state and federal documentation requirements and compliance.

Education

Master’s degree in social work

ACM Social Worker certification

BLS Certification (American Heart Association)
