Job summary

Qualifications

• Master's degree in Social Work.
• 1 year of experience.
• TX licensure: LCSW/LMSW/LMSW-AP or equivalent required.

Responsibilities

• Conducts psycho-social assessments and documents findings.
• Develops and updates treatment plans with patient and family education.
• Provides or coordinates care plans including crisis intervention and discharge planning.
• Collaborates with healthcare team to ensure appropriate resource placement.
• Participates in performance improvement initiatives and data collection.
