Job summary

VNS Health in New York City seeks a Psychiatric Social Worker for the Children's Mobile Crisis program. You will perform psychosocial assessments, develop short-term plans, and provide counseling and care management across a multidisciplinary team to ensure continuity of care.

The role requires MSW and LMSW licensure in NYS, with bilingual abilities a possible plus. This program delivers immediate crisis response for children and adolescents with growth opportunities and CEU credits.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Current registration to practice as a Licensed Master Social Worker in New York State.
• Bilingual abilities may be required as per operational needs.

Responsibilities

• Screen telephone referrals and perform psychosocial evaluation.
• Develop and implement short-term service plans for patients and families.
• Provide counseling and care management with referrals to long-term services.
• Coordinate with interdisciplinary team to ensure continuity of care.
• Maintain case records per program policies.
• Assist and collaborate with Program Coordinator as needed.
• Provide outreach to mentally ill individuals in the community.
