Job summary

Qualifications

• Valid Driver's License required.

Responsibilities

• Completes clinical summaries and maintains thorough charting as per policy.
• Collaborates with interdisciplinary team to plan and revise the care plan.
• Identifies and coordinates appropriate community resources and discharge planning.
