Job summary

Enhabit Home Health & Hospice is seeking a licensed master's level social worker to provide intermittent professional services to patients at home. You will educate patients and families, collaborate with the health care team, and follow physician orders under the plan of care.

The role requires an MSW, state licensure, and at least one year of health-care social work experience; Medicare/home health exposure is a plus. Valid driver’s license and CPR are required.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Must be certified or licensed in the state where practicing.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
