Riverside Health System in Gloucester, Virginia seeks a qualified Medical Social Worker (MSW) to provide therapeutic services under physician supervision. The role involves psychosocial assessment, care planning, and family education for patients and their families facing terminal illness.

The position requires an MSW, clinical internship or 2 years of experience, CPR/BLS certification, and a valid driver's license. LCSW is preferred.

Qualifications

• Masters Degree, Social Work (Required)
• Clinical internship in a hospital or residential setting, or two years clinical experience (Preferred)
• CPR/BLS Certification (Required)
• Licensed Clinical Social Worker (LCSW) - Virginia (Preferred)
• Valid Driver's License Required

Responsabilités

• Provides therapeutic services for patients, residents, and their families.
• Accurately assesses biopsychosocial functioning and synthesizes information for treatment planning.
• Completes Diagnostic Summaries and maintains thorough medical record documentation.
• Participates in interdisciplinary team meetings and plan of care development.
• Assists in transferring responsibilities regarding fiscal/legal and healthcare decisions.
• Identifies community resources and supports patient/family in discharge planning.
