Job summary

Riverside Health System seeks a licensed social worker (MSW) to provide therapeutic services to patients, residents, and families under physician supervision. The role includes psychosocial assessments, care planning, and collaboration with the interdisciplinary team to support patient care needs.

You will perform counseling, crisis intervention, and preparation of advanced directives, while documenting case management and ensuring quality care within the plan of care.

Qualifications

• Master's degree in Social Work required
• Clinical internship in a hospital or residential setting or two years clinical experience preferred
• CPR/BLS certification required within 30 days
• Licensed Clinical Social Worker (LCSW) preferred
• Valid driver's license required

Responsibilities

• Provides therapeutic services for patients, residents, and their families
• Accurately assesses biopsychosocial functioning using interviewing, observation, and assessment skills
• Synthesizes information into notes useful for treatment planning
• Completes Diagnostic Summaries as assigned
• Maintains thorough medical record documentation according to policy
• Participates in interdisciplinary team meetings and communicates patient needs to the team
• Provides education for families on suicidality, diagnosis, and plan of care
