Compassus is seeking a Home Health Licensed Master Social Worker (LMSW) to provide psychosocial assessments, counseling, and referrals to patients of all ages in the home setting. The role supports Compassus values and the 6 Pillars of Success while promoting coordination of care and ethical practice.

The LMSW will collaborate with care teams, assess needs, document progress, and ensure alignment with care plans. A strong emphasis is placed on patient advocacy and community resource linkage.

Qualifikationen

• Master’s degree in Social Work required.
• Active LMSW/LCSW licensure required (state dependent).
• Strong written and verbal communication skills.

Aufgaben

• Provides care using infection control measures and adheres to OSHA guidelines.
• Maintains professional growth by participating in required in‑services.
• Accepts clinical assignments appropriate to education and competence.
• Monitors cases for third-party payor requirements and approves appropriate referrals.
• Communicates effectively with patient/family, team members, physicians, and other health professionals to coordinate services.
• Ensures continuity of quality patient care per plan of care with proper documentation.
• Assesses psychosocial, financial, environmental, and community needs; documents findings and coordinates resources.
• Performs other duties as assigned.
