Job summary

Trinity Health PACE's LIFE at St Francis Riverfront in Wilmington, DE seeks a Social Worker (MSW) to plan, implement and coordinate social services for participants and families, including case management, counseling and advocacy.

This full-time role operates Mon–Fri, 8:00 am–4:30 pm with no nights or weekends; requires DE-licensed MSW, strong communication, and driving ability. It involves work in the facility, occasional home visits and hospital/nursing home visits, and collaboration with IDT.

Qualifications

• Master’s degree in Social Work required.
• Two years of experience.
• Current DE social work license required.
• Experience with frail or elderly population preferred.
• Experience on multidisciplinary medical/community-based team preferred.
• Knowledge of Medicare, Medicaid and other payment systems.
• Valid driver's license, insurance and use of personal vehicle.
• Superior written and verbal communication skills.
• Proficiency with MS Office.
• Ability to manage conflict and adapt to change.

Responsibilities

• Assess participants and coordinate care plans with IDT.
• Conduct intake meetings, provide counseling and support.
• Act as liaison with agencies and provide resource referrals.
• Coordinate discharge planning and transitional care.
• Provide individual and family counseling.
• Facilitate discharge placement planning and liaison between participant and agencies.
